Perhaps selfishly, he was always too preoccupied with deflecting worry from himself, and in turn he's tried to give those he cares about a certain amount of the faith he wasn't always handed freely. What fears he has he prefers to meet head on, with action one way or another.
There hasn't been much he could do for Bucky until very recently, unless you count indirect ways like taking out the remains of Hydra, which he would've done regardless. Barring that, all he's really had is worry, and no small amount of regrets. It's stuck, like the guilt and the grief.
